A completed form can be returned via one of the following methods: mail to IBT of PORAC, 2960 Advantage Way, Sacramento, CA 95834; fax to (916) 999-8892; or email to lifeanddisability@ibtofporac.org.

If you have questions regarding the application, please call the IBT at (800) 655-6397 and ask to speak with the Disability Administrative Assistant.

Your claim will not be processed until all completed forms and requested documents are received by the IBT.
